We are accustomed to dealing with people who are in various stages of need for our services. Our Admissions Team is available to answer all of your questions from 8:30 a.m. to 5:00 p.m. Monday through Friday, or by appointment.

In order for us to anticipate your needs before your admission, we highly recommend your loved ones tour our facility. We feel confident that our facility is presentable and staff is available to assist you with your questions and concerns at any time. Tours also can be arranged after normal business hours and on weekends.

In order for the admissions process to go smoothly and quickly, our staff would need the following:

• Patient's medical history and physical from the hospital or doctor
• List of current medications from the hospital or doctor
• Mansfield Memorial's completed financial application
• All insurance and Medicare information
• A physician designated to follow your case (which we can assign if necessary)
• If your goal is to return home after rehabilitation, discharge planning begins the day of admission
